The top section of forms are all in Adobe Acrobat PDF format for cross platform compability. They can all be filled out online and then either e-mailed (you cannot save and e-mail a form if you only have the free adobe reader) to the office or printed and faxed (315-463-5729) or delivered.

To e-mail your form follow these simple instructions: This will work only if you have a save function in your program.
1.) When the form opens in your Adobe Acrobat window, from the File dropdown menu select the Save as option and select a directory or the desktop to save your file. You may change the name of your file if you wish.

2.) You can fill out the form before or after you save it. To fill it out after saving open the file you saved and fill in the blanks. When you are done save it again with the changes. You can then e-mail it to anyone as an attachment. If you need e-mail addresses for the office they are available on this site.

To fax forms from your computer:
1.) Completely fill out the form and double check it for accuracy.

2.) Use the print command in your viewer and select your built in fax software as your printer. Follow the instructions for your software and fax to the office at 315-463-5729 (there is no toll free fax line).

3.) If you only have one phone line you have to break your connection before you can send a fax. Keep your browser window open so you don't lose the data on the form. If you have RoadRunnder or another non-telephone internet connection this isn't a problem.
